Despite their seemingly light-hearted name, marionette lines refer to a facial area that is often disliked by patients and can be challenging to treat. These are the lines that run from the corners of the mouth down toward the chin. They are caused both by the natural loss of tissue elasticity associated with aging and by the gravitational pull that drags down the mid-face over time.

To achieve a natural-looking result, treatment should begin with the zygomatic (cheekbone) area. By lifting the mid-face, we reduce the downward pressure and visual weight affecting the marionette lines.
After this first step, the treatment can be refined by injecting filler directly into the affected area to soften or eliminate the shadow caused by the folds.

An alternative and effective approach involves horizontal, multi-level injections in the marionette zone. This technique avoids further heaviness in the area and provides a more natural and harmonious result. If needed, the outcome can be enhanced with a combined treatment approach, always tailored to the patient's anatomy and aesthetic goals—especially avoiding excessive volume in the cheeks if that is not the patient’s desire.
The ultimate goal is always to preserve naturalness and facial balance while precisely meeting the patient's expectations.
